| Account Information Release Authorization | The following two forms will authorize ComEd to release to you or your agent customer information to the extent it is available. |
| Return to Bundled Service | This form allows a non-residential customer receiving electric service on Rate RDS to return to the applicable ComEd Basic Electric Service bundled rate. There is a minimum seven (7) calendar day lead time notice required for this form. |
| Designation of General Account Agent | This form allows a non-residential customer to designate an agent to arrange and manage services provided by ComEd. |
| Agency Termination | An agent or a customer must use this form to terminate an agency relationship that has been established. A signature is required as validation for this form. |
| BES-H (Hourly) Election Form | This form allows a non-residential customer to elect service under BES-H (Hourly). There is a minimum seven (7) calendar day lead time notice required for this form. |
| BES-H (Hourly) Termination Form | This form allows a non-residential customer to terminate service under BES-H (Hourly). There is a minimum seven (7) calendar day lead time notice required for this form. |
| Return to Bundled and Elect BES-H (Hourly) Form | This form allows a non-residential customer receiving electric service on Rate RDS to return to the ComEd BES-H (Hourly) bundled rate. There is a minimum seven (7) calendar day lead time notice required for this form. |
| Unblocking Form | To remove a block placed on an account, fill out the form and mail to the specified mailing address or fax to the specified number. |
